Dear Cheyenne,

All students must satisfy medical clearance requirements, including immunization requirements, to be medically cleared for enrollment at UAB. You are receiving this reminder because you have not yet satisfied all of your requirements. You must satisfy these requirements by August 1, 2016 to avoid an adverse effect on your fall registration.

To submit your immunization documents, log into BlazerNET using your Blazer ID and password and click on “Patient Portal Online Appointments”.

Students may receive vaccinations and TB testing at UAB Student Health & Wellness. Please schedule an appointment using the “Patient Portal Online Appointments” button on your BlazerNET home page or by calling 205-934-3581
